Congregational Care Consent Agreement

Unity on the Bay provides spiritual support (Spiritual Counseling, Coaching and/or Spiritual Direction) to those persons requesting it. Spiritual Support is based upon our belief that there is One Presence and One Power in the universe, God the Good, Omnipotent, and that this Power manifests in our lives as spiritual principle and as our personal, loving Comforter. Spiritual Support assists the person in finding and hearing God as God reveals itself to that individual.

Spiritual Support is conducted in a professional manner and in alignment with our Unity Worldwide Ministry’s Code of Ethics and Sexual Conduct Policy. A spiritual support session lasts about one hour. Each congregant can requests up to three (3) session. They are conducted at our offices during our normal hours and/or at reasonable hours when other staff members are present in the building.

Your Spiritual Support Facilitator is trained to assist you in applying spiritual principles to the concern you have brought for support. Spiritual support is not psychotherapy. If you desire psychotherapeutic help and/or need the assistance of additional social services, we may be able to provide you a list of possible resources. These services are neither recommended, endorsed, nor opposed by Unity on the Bay.

As your spiritual counselor, I will honor the confidentiality of what you say in the spiritual support session, with the exception of the safety concerns listed next. If you tell me that a child or dependent adult has been harmed or is at risk of harm, or that you may hurt yourself or someone else, I will share that information with the appropriate authorities who can intervene. This information will be disclosed according to state mandates for reporting child, elder, and disabled or dependent adult abuse and according to the ethical standards of practice for ministry professionals. Confidentiality will be maintained about any and all information not directly related to the concern for safety.

Spiritual Support is provided on a Love Offering basis. Our suggested Love Offering is $75 per session.
